Skin suturing

Contra-indications

      • Risk of infection is high (e.g. dog bites, heavily contaminated wounds, gun shot injuries)
      • Risk of dehiscence is high due to ischaemia  (high tissue tension)
 Methods of approximation

        • Sutures
        • Staples
        • Sterile plasters (steri strips)
        • Tissue glue
Pre-requisites
  • Obtain consent
  • Adequate anaesthesia
  • Clean the wound
Instruments

  • Toothed forceps
  • Needle holder
  • Appropriate suture
Technique

  • Hold the needle with needle holder
  • Lift the edge of the skin using the toothed forceps
  • Enter the skin at 90 degrees and exit through the wound
  • Enter the opposite wound surface and exit at 90 degrees to the skin surface. Press on the skin to apply counter pressure to help exiting the needle
  • Tie a square knot
  • Repeat the procedure ( the distance from the edge of the wound should correspond to the thickness of the tissue and successive sutures should be placed at twice this distance apart) - figure
  • Ensure that all knots are on the same side of the cut edge
